Hi Margaret, Yes it does come with the mattress. Unfortunately the mattress has a stain on it that, I believe, was caused by a cup of tea rather than anything more unsavoury. For this reason I didn't add it to the original description. The bed does not have a headboard. Hope this helps. Kind regards. David
